ORP Statement for Social Security

ORP Administrator’s Statement: The ORP Plan Administrator provides a statement of your participation in the ORP; your account balance on the day you terminated employment with the Commonwealth; and a confirmation of your ORP Provider for you.

You must present the Statement to the SSA Representative who submits your application for “Old Age” benefits.

The ORP Plan Administrator routinely sends these statements to participants who have terminated employment and attained age sixty-two (62).

Contact the Plan Administrator: You must contact the Plan Administrator to request your Statement if you are applying for Social Security benefits and have not yet received one.
